Abstract: This study was designed to assess the laboratories microscopic diagnosis of malaria parasites in Khartoum State and Butana Locality, Gezira State. The protocol includes the comparison of thirty laboratories (Hospitals, Health centres and private laboratories) 13laboratoreis from Khartoum State and 17 laboratoreis from Butana Locality, Gezira State. The data from every laboratory were recorded in a questionnaire including the general appearance of the laboratory the building, area, variation and the qualification and experience of the working staff. It also includes equipment, microscopes, sample processing, kind of film, preparation and storage of stains used and finally reporting of the results. Two blood film samples were taken From(30) laboratories(584)patients one processed with Giemsa Stain (10%) for the study and the other for the checkedlaboratory.From seven laboratories in Khartoum State, three samples were taken from( 253) patients, one was processed using Giemsa Stain and the other one was routinely done at the laboratory visited (Hospital, Health Center and Private Laboratories) the third to the reference laboratory. The results obtained were compared. The results were evaluated as malaria (+ ve) or (–ve) and compared with my observations. Significant differences were noticed where more (+ ve) blood samples were recorded, compared to the checked laboratories (Hospitals, Health Centers and Private Laboratories), (p>0.05). In Khartoum State laboratories, the differences were not significant from the study results (p< 0.05). These differences are due to the preparation of the blood films, stain used, quality of microscope, condition of laboratories or the experience and qualifications of the microscopists
