Title: Impact of Clinical Pharmacist Intervention in Warfarin Therapy among Sudanese Patients in Ahmed Gasim Hospital

Abstract: Warfarin is the anticoagulant of choice when long-term or extended anticoagulation is required, but it is the third on the list of drugs implicated in causing hospital admission through adverse effects. Clinical pharmacists have a great role in anticoagulation therapy, and have been involved in warfarin therapy management in the USA and Europe since the 1970s. The objective of this work is to assess the impact of clinical pharmacist intervention in maintaining therapeutic (INR) effect, reducing adverse effects and hospitalization, improving patient knowledge and adherence to warfarin therapy.
